The base notes pairs are strummed. For the D chord you should think 'C chord two frets up the neck'. Don't try to finger the full Em, just what is needed. You'll need your pinky for the hammer-on, etc. You have to get the pacing/rythme by repeated listening to the recording. The embellishments in the second version are not found in the 2x of the intro, but they are there in some online recordings at some point during the song. There are lots of things that can be done including hammering on the B string notes on the Am and D chords. I think I hear that at some point too.
